1. In order to apply for a pharmacist's professional qualification certification examination, the pharmacist must submit an application accompanied by the following documents:
(a) a copy of the document certifying higher pharmaceutical education (presenting the original);
(b) a copy of the document certifying the change of surname (given name), if the surname (given name) has been changed (presenting the original);
(c) a statement issued by the employer (original) certifying professional activity in a speciality during the last three years;
(d). a copy of the document (presenting the original) attesting the proficiency of the official language of the pharmacist at the highest level in conformity with the requirements of the National Language Law, if the pharmacist has obtained general secondary or higher education in a language other than Latvian;
(e). a statement signed by the pharmacist (original) regarding compliance with the requirements referred to in Section 58, Paragraph one, Clauses 2 and 3 of the Pharmaceutical Law;
F). two photographs (3.5 x 4.5 cm).
2. For re-certification, the pharmacist shall submit an application, accompanied by the following documents, two months before the expiry of the term of validity of the certificate:
(a) a statement issued by the employer (original) certifying professional activity in a speciality during the last five years;
(b) a statement signed by the pharmacist (original) regarding compliance with the requirements referred to in Section 58, Paragraph one, Clauses 2 and 3 of the Pharmaceutical Law;
(c) two photographs (3,5 x 4,5 cm);
(d) copies of documents certifying the development of professional qualifications (request for copying of certificates on both sides of the page; presentation of originals);
(e) a report on the development of professional qualifications in continuing training activities (the report can be printed from the closed section OF THE LFB site for each individual, either in the drafting process or IN THE LFB office, providing documents for re-certification);
Documents may be submitted in person, electronically and by post. When requesting a service in person, a personal identification document (passport or identity card) must be produced. An application, statement, employer's statement and report sent by e-mail must have been signed with a secure electronic signature accompanied by a time stamp.
